I have developed a form that you can download and enter in the name of the person that you are standing in faith for a healing.  As you read it you can see where to enter the name of the person and the issue that they are dealing with.  Below is the form in a word document and a pdf form.  Please feel free to download this to your computer and use it as a declaration of your faith.
